Good Samaritan Hospice in Roanoke, VA seeks a Registered Nurse Case Manager for Home Health to coordinate, implement, and evaluate patient care in the home setting in accordance with physician orders and individualized plans of care.

The RN will educate patients and families, lead care decisions with the interdisciplinary team, and ensure quality outcomes while maintaining independence and safety. A valid VA RN license and BLS are required.

Qualifications

• Graduate of an accredited nursing program.
• Virginia RN license (or compact license).
• Active BLS/CPR certification or obtained within 30 days of hire.

Responsabilités

• Conduct skilled nursing visits in patients' homes.
• Complete admissions, re-certifications, and discharge assessments.
• Develop, implement and revise Plans of Care based on patient needs.
• Complete OASIS documentation accurately and on time.
• Coordinate care with physicians, therapists, and the interdisciplinary team.
• Provide patient and caregiver education on disease process, medications, and self-care.
• Monitor patient progress and adjust care plans as needed.
• Participate in case conferences, on-call rotation, and administrative meetings.
