Job summary

Affinity Hospice Management LLC seeks an experienced Registered Nurse to direct hospice care in home settings in Roanoke, VA. You will assess patients, develop and adjust plans of care, administer treatments, and collaborate with the interdisciplinary team to meet patient and family needs.

The role emphasizes community health education, caregiver coaching, and ensuring a safe care environment in patients' homes, while complying with state regulations and quality standards.

Qualifications

• RN licensed in Virginia with good standing.
• One year of professional nursing experience within the last three years OR a bachelor’s degree with two years in public health/home care/hospice is preferred.

Responsibilities

• Complete initial and ongoing assessments to determine hospice needs.
• Develop and revise care plans with patient and family involvement.
• Provide nursing care per the nursing process and physician plans.
• Coordinate with physicians and the interdisciplinary team.
• Participate in on-call duties and ensure equipment and services are available.
• Support and supervise ancillary personnel as needed.
