Job summary

Affinity Hospice Management LLC in Roanoke, VA is seeking an experienced registered nurse to plan, organize and direct hospice care in patients' homes and communities. The RN will lead assessment, care planning and coordination with the interdisciplinary team to meet patient and family needs.

Qualifications

• Graduate of an accredited school of nursing.
• Licensed Registered Nurse in good standing.
• 1 year of experience within the last 3 years; or bachelor’s degree with 2 years' nursing experience in public health, home care, or hospice preferred.
• Management experience not required; may supervise hospice aides.
• Contingent upon satisfactory references and background checks.
• Excellent observation, verbal and written communication, problem solving and basic math skills.
• Prolonged standing and ability to lift/transfer patients and equipment.
• Licensed driver with a vehicle in good working order.
• CPR certification required.

Responsibilities

• Completes initial, comprehensive assessment of patient and family to determine hospice needs.
• Provides professional nursing care by utilizing nursing process.
• Assesses and evaluates patient status; writes plan of care and revises as needed.
• Initiates plan of care and revises as patient status changes.
• Uses health data to determine nursing diagnosis.
• Develops care plans with patient/family involvement.
• Administers medications and treatments as prescribed.
• Counsels patient and family in meeting nursing needs.
• Provides health care instructions to patient as appropriate.
• Assists patient with activities of daily living and supports self-care and comfort.
• Acts as Case Manager when assigned by Clinical Supervisor.
