Kindful Health LLC is seeking a Patient Care Coordinator (RN) to ensure compassionate, patient-centered hospice care. You will report to the Director of Clinical Services and oversee coordination to meet quality and regulatory standards.

The role requires strong organizational skills, clinical oversight of nursing staff, and collaboration with physicians, social workers, chaplains, and therapists to maintain effective care plans.

Formación

• BSN preferred; ADN with relevant hospice experience will be considered.
• Current Registered Nurse (RN) license in Texas.
• CHPN certification preferred.
• Minimum of 2 years nursing experience, preferably in hospice or palliative care.
• Experience in care coordination, case management, or supervisory roles in a similar role is preferred.

Responsabilidades

• Coordinate and monitor the day-to-day delivery of patient care services, ensuring that all aspects of the patient’s care plan are carried out effectively.
• Collaborate with interdisciplinary team members to develop and update individualized care plans.
• Act as a liaison between patients, families, and the hospice care team to ensure clear communication.
