The Patient Care Coordinator (PCC) is a vital member of the hospice care team, responsible for ensuring the delivery of compassionate, patient-centered care to hospice patients and their families. This role reports to the Director of Clinical Services and involves overseeing and coordinating clinical services to ensure quality, continuity, and consistency of care in alignment with hospice philosophy and regulatory requirements.
Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) with relevant experience will be considered . Current Registered Nurse (RN) license in the state of Texas. Certification in Hospice and Palliative Nursing (CHPN) preferred. Minimum of 2 years nursing experience, preferably in hospice or palliative care. Experience in care coordination case management, or supervisory roles in a similar role is preferred.
Strong organizational and time management skills, with the ability to handle multiple tasks efficiently. Excellent communication and interpersonal skills, with a compassionate and patient-focused approach. Proficiency in electronic medical records (EMR) and other healthcare-related technologies. Proficient in running, analyzing, and interpreting reports to identify trends and insights. Able to leverage data to inform and implement practice changes, enhancing patient care outcomes. Strong ability to use data-driven decision-making to support continuous improvement in hospice care services.
