Job summary

Priority Dispatch Corp. in Austin, TX is seeking a Patient Care Coordinator (PCC) to lead and coordinate hospice nursing services, reporting to the Director of Clinical Services. This role emphasizes care coordination, clinical oversight, and restoring patient comfort and dignity in end-of-life care.

Qualifications

Responsibilities

  • Coordinate and monitor day-to-day delivery of patient care services, ensuring care plans are carried out effectively.
  • Collaborate with physicians, social workers, chaplains, and therapists to develop/update individualized care plans.
  • Act as liaison between patients, families, and the hospice care team to ensure clear communication.
  • Provide clinical supervision to nursing staff (LPNs and CNAs) and ensure adherence to policies.
  • Participate in case conferences, clinical meetings, and quality improvement initiatives.
  • Assist in admissions, discharges, transfers, and documentation to ensure regulatory compliance.
  • Educate patients and families about hospice care processes, medications, and symptom management.
  • Support ongoing education and training for hospice staff.

Job Summary:

The Patient Care Coordinator (PCC) is a vital member of the hospice care team, responsible for ensuring the delivery of compassionate, patient-centered care to hospice patients and their families. This role reports to the Director of Clinical Services and involves overseeing and coordinating clinical services to ensure quality, continuity, and consistency of care in alignment with hospice philosophy and regulatory requirements.

Essential Education and Experience:

Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) with relevant experience will be considered . Current Registered Nurse (RN) license in the state of Texas. Certification in Hospice and Palliative Nursing (CHPN) preferred. Minimum of 2 years nursing experience, preferably in hospice or palliative care. Experience in care coordination case management, or supervisory roles in a similar role is preferred.

Responsibilities & Duties:
  • Care Coordination:
    • Coordinate and monitor the day-to-day delivery of patient care services, ensuring that all aspects of the patient’s care plan are carried out effectively and efficiently.
    • Collaborate with interdisciplinary team members, including physicians, social workers, chaplains, and therapists, to develop and update individualized care plans.
    • Act as a liaison between patients, families, and the hospice care team to ensure clear communication and address any concerns or needs promptly.
  • Clinical Oversight:
    • Provide clinical supervision and support to nursing staff, including licensed practical nurses (LPNs) and certified nursing assistants (CNAs), ensuring adherence to hospice policies, procedures, and best practices.
    • Participate in case conferences, clinical meetings, and quality improvement initiatives.
    • Assist in the management of patient admissions, discharges, and transfers, ensuring accurate documentation and regulatory compliance.
  • Patient and Family Support:
    • Offer emotional support and guidance to patients and their families, helping them navigate the end-of-life journey with dignity and respect.
    • Educate patients and families about the hospice care process, medications, and symptom management.
  • Quality Assurance:
    • Assist the Director of Clinical Services in monitoring and maintaining high standards of care, identifying areas for improvement, and implementing corrective actions as needed.
    • Ensure compliance with all state, federal, and accrediting agency regulations, including proper documentation and reporting.
  • Education and Training:
    • Provide ongoing education and training to hospice staff on clinical care practices, hospice philosophy, and regulatory requirements.
    • Participate in professional development opportunities to stay current with best practices in hospice care.
Qualifications & Skills:

Strong organizational and time management skills, with the ability to handle multiple tasks efficiently. Excellent communication and interpersonal skills, with a compassionate and patient-focused approach. Proficiency in electronic medical records (EMR) and other healthcare-related technologies. Proficient in running, analyzing, and interpreting reports to identify trends and insights. Able to leverage data to inform and implement practice changes, enhancing patient care outcomes. Strong ability to use data-driven decision-making to support continuous improvement in hospice care services.
