Job summary

Kindful Health LLC seeks a Patient Care Coordinator to lead care coordination for hospice patients and their families. The PCC reports to the Director of Clinical Services and ensures quality, continuity, and compassionate care in alignment with hospice philosophy and regulations.

Qualifications include a Texas RN license, BSN preferred, CHPN preferred, and 2+ years in hospice or related fields. The role emphasizes clinical oversight, care plan updates, and education for staff and families.

Qualifications

• Bachelor of Science in Nursing preferred; ADN with relevant experience will be considered.
• Current RN license in Texas.
• CHPN certification preferred.
• Minimum 2 years nursing experience, preferably in hospice or palliative care.
• Experience in care coordination or supervisory roles preferred.

Responsibilities

• Coordinate and monitor day-to-day patient care delivery and ensure adherence to care plans.
• Collaborate with physicians, social workers, chaplains, and therapists to update care plans.
• Act as liaison between patients, families, and the hospice care team for clear communication.
• Provide clinical supervision to LPNs and CNAs, supporting hospice policies and best practices.
• Assist in admissions, discharges, transfers, and regulatory documentation.
• Offer emotional support and educate patients and families about hospice care.
