Job summary

Kindful Health in Spring, TX seeks a Patient Care Coordinator to lead patient-centered hospice care coordination. You will oversee daily care delivery, align care plans with the inter-disciplinary team, and ensure clear communication with patients and families.

BSN preferred; ADN with relevant experience considered. Texas RN license required; CHPN preferred. You will supervise nursing staff, participate in case conferences, drive quality improvement, and ensure regulatory compliance.

Qualifications

• Bachelor of Science in Nursing preferred; ADN with relevant experience will be considered.
• Current RN license in Texas required.
• CHPN certification preferred.
• Minimum 2 years nursing experience, preferably in hospice or palliative care.

Responsibilities

• Coordinate and monitor day-to-day delivery of patient care services.
• Collaborate with interdisciplinary team to develop and update care plans.
• Act as liaison between patients, families, and hospice team to address needs.
• Provide clinical supervision to nursing staff and participate in quality initiatives.
• Assist in admissions, discharges, transfers, and ensure regulatory compliance.
