Job summary

Chapters Health is seeking an Admission RN to assess patients in residences or hospital settings and initiate care through the company services. This role acts as an ambassador for the organization and facilitates a seamless transition to Care Navigation.

Applicants should hold an RN license in the working state, have at least 1 year of nursing experience (hospice or hospital preferred), and be proficient with EMR/EHR systems. Bilingual Spanish is a plus; valid driver's license required.

Qualifications

• Current RN license in the state where you will work.
• Minimum 1 year nursing experience; hospice or hospital preferred.
• Experience using EMR/EHR systems.
• English communication; bilingual in Spanish preferred.
• Must be able to handle multiple priorities and stress.
• Valid driver's license and reliable transportation.

Responsibilities

• Complete clinical admissions for Chapters Health services.
• Explain services to patients/families and address questions empathetically.
• Obtain physician orders and initiate Plan of Care.
• Coordinate with Care Navigation for timely follow-up and referrals.
• Maintain documentation per company policy and SBAR communications.
• Attend to after-hours needs as required.
