Job summary

Chapters Health System is seeking an Admission RN to assess patients in their residence or hospital and initiate care with Care Navigation. The role requires an active RN license and at least 1 year of nursing experience, hospice or hospital preferred, plus EMR/EHR familiarity. Bilingual Spanish is a plus.

You will collaborate with facilities, ensure proper admissions and documentation, and support discharge planning while maintaining patient- and family-centered care.

Qualifications

• Current RN license in the state where work will occur.
• Minimum of one year nursing experience; hospice or hospital experience preferred.
• Experience with EMR/EHR (Electronic Medical/Health Record).
• Strong clinical assessment skills; ED/ICU background preferred.
• Ability to communicate in English (verbal and written); bilingual in Spanish preferred.

Responsibilities

• Complete clinical admissions for all Company services; explain services to patients and families with empathy.
• Identify admission objections and provide responses.
• Educate patient/family on admission requirements and initial needs.
• Complete consents and initial assessment per regulations and policy; coordinate with physicians.
