Job summary

Residential Home Health and Hospice seeks an experienced RN for a Hospice position in the Leawood, KS area. The role involves delivering end-of-life care at patients' homes, collaborating with an interdisciplinary team for comfort and quality of life, and performing comprehensive assessments.

A $10,000 first-year sign-on is offered to reward milestone achievement. Active RN license in the state, two years of RN experience (hospice preferred), and a valid driver’s license with travel to patient

• Two years of RN experience; Hospice experience preferred.
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.

Responsibilities

• Deliver compassionate, patient-centered care in hospice settings.
• Perform comprehensive patient assessments, symptom and pain management, and end-of-life care planning.
• Educate and support patients and families through the end-of-life journey.
• Think critically, document thoroughly in EMR, and collaborate with the interdisciplinary team.
• Work independently in patient homes, traveling within the assigned territory.
• Maintain compliant documentation and ensure quality of care.

• Work independently, but never alone
