Job summary

Residential Home Health and Hospice in the Chicago area seeks an experienced RN to provide compassionate hospice care in patients' homes. You will work with an interdisciplinary team to support end-of-life care, ensuring comfort, dignity, and quality of life.

Requirements include an active RN license in the state, a minimum of two years of RN experience (hospice preferred), and a valid driver’s license with ability to travel to patient locations.

Qualifications

• Active Registered Nurse (RN) license in the state you work
• Two years of RN experience; hospice experience preferred
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• Perform comprehensive patient assessments, symptom and pain management, and individualized end-of-life care planning
• Educate and support patients and families through the end-of-life journey, ensuring they feel informed, prepared, and deeply cared for
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory

RN Qualifications
