Job summary

ExcelaCare Hospice - Lubbock, TX is seeking an RN to provide compassionate hospice care in clients' homes, skilled facilities, or assisted living settings. You will work with an interdisciplinary team to develop and implement a patient-centered Plan of Care and educate families.

Responsibilities include administering medications, documenting patient status, coordinating with physicians and caregivers, and participating in on-call rotations. Travel is required and independence is essential.

Qualifications

• Graduate of an accredited school of nursing.
• Two years RN experience; hospice preferred.
• Valid RN license and CPR certification required.
• Valid driver’s license and auto liability insurance.

Responsibilities

• Administers hospice care per the Plan of Care and physician orders.
• Coordinates care with a multidisciplinary team and families.
• Documents patient status accurately and timely.
• Participates in on-call rotation and travels to patient locations.
• Educates patients and families on care plans and options.
