Job summary

ExcelaCare Hospice – Lubbock, TX seeks an experienced Registered Nurse for weekend on-call coverage. You will administer hospice care in patients' homes or facilities, coordinate with the care team, and respond to needs after hours.

The role emphasizes independent work, strong assessment skills, documentation, patient-family education, and collaboration with physicians. Licensure and CPR required; travel locally; some on-call rotations.

Qualifications

• Graduate of an accredited school of nursing.
• Two years RN experience; hospice preferred.
• Current RN license and CPR certification.
• Valid driver’s license and auto liability insurance; no OIG exclusions.

Responsibilities

• Apply nursing knowledge to patient care using the nursing process.
• Administer meds and treatments per policy and guidelines.
• Perform assessments and develop a Plan of Care with physician orders.
• Document patient status timely and accurately; update care plan.
• Build strong relationships with referring physicians and families.
• Collaborate with interdisciplinary team on care plans.
• Educate patients and families on care options.
• Attend on-call rotation and complete scheduled visits.
