High Plains Senior Care Hospice is seeking a compassionate and dedicated Full-Time Registered Nurse (RN) to provide hospice care to patients in their homes in Lubbock, TX. The RN will perform assessments, coordinate with the interdisciplinary team, educate patients and families, and ensure comfort-focused, patient-centered care throughout the hospice journey.

Qualifications

• Current RN license in Texas is required.
• Minimum 1 year RN experience is required.
• Home Health or Hospice experience is preferred.
• Valid driver's license, reliable transportation, and current auto insurance.

Responsabilités

• Perform initial and ongoing comprehensive patient assessments.
• Develop, implement, and update individualized care plans.
• Provide skilled nursing care and symptom management for hospice patients.
• Educate patients and families on disease progression, medications, self-care, and end-of-life care.
• Coordinate care with physicians and the interdisciplinary hospice team.
• Monitor patient condition and communicate changes to the physician and care team.
• Complete accurate and timely clinical documentation and progress notes.
• Assess caregiver needs and provide education and support.
• Refer to community resources and support services as needed.
• Participate in case conferences and collaborate with the team.
• Provide on-call coverage as assigned.
• Follow agency policies, state regulations, and hospice standards of care.
