Job summary

ExcelaCare Hospice in Lubbock, TX is seeking a dedicated RN Case Manager to administer hospice care at patients’ homes, facilities, and healthcare settings. You will coordinate with the interdisciplinary team, family, and referring agencies to implement individualized care plans and ensure high quality end‑of‑life support.

The role requires RN licensure, CPR, and willingness to travel; on‑call responsibilities are part of the schedule.

Qualifications

• Graduate of an accredited school of nursing.
• Two years of RN experience in a clinical setting; hospice experience preferred.
• Current valid RN license, CPR certification, driver’s license, and auto liability insurance.

Responsibilities

• Administers medications and treatments according to Agency policy.
• Performs accurate assessments and develops a written Plan of Care with physician orders.
• Coordinates care with the interdisciplinary team and patient/family; communicates needs.
• Documents patient status accurately and timely; notes interventions and response.
• Travels locally and participates in on-call rotation as required.
• Educates patients and families on healthcare plan, symptom control, and end‑of‑life options.
