Partners In Care (PIC) in Bend, Oregon seeks a Support Registered Nurse to plan, coordinate, and evaluate hospice services with cross-training in home health. The RN provides cost-efficient, high-quality patient care in patients' homes and facilities throughout Central Oregon.

Qualifications

• Unencumbered Oregon RN License.
• Ability to pass a background test, including a Motor Vehicle Report (MVR), drug screen, and TB test.
• Graduate of an accredited nursing school.
• One to two years of recent medical care experience in an institutional setting is preferred.
• Current automobile insurance is required.
• A valid Oregon driver’s license and reliable transportation.
• Valid CPR Certification.
• Strong communication and interpersonal skills.
• Ability to work independently and collaboratively within an interdisciplinary team.
• Compassion, empathy, and dedication to patient care.

Responsibilities

• Provide initial comprehensive assessments and determine home health needs.
• Develop an individualized care plan with goals based on nursing diagnosis.
• Provide nursing procedures and treatments and administer medications as prescribed.
• Update ongoing assessments and care plans as patient status changes.
• Coordinate patient care with case manager and other healthcare team members.
• Develop and monitor the HCA plan of care and complete supervisory visits every 14 days.
• Communicate with physicians and facility staff; document coordination of care.
• Complete required reports and documentation within specified timeframes.
