Partners In Care in the United States seeks a Support Registered Nurse to plan, coordinate, implement, and evaluate hospice services with cross-training in home health. The role emphasizes nursing expertise, community health education, and cost-efficient patient care across patients’ homes.

Ideal candidates have 1–2 years of relevant care experience, Oregon RN license, CPR, and strong communication. Travel throughout Central Oregon is required, with 8:00–5:00 hours, Monday–Friday, full-time

Qualifications

• Unencumbered Oregon RN license.
• Background check including MVR, drug screen and TB test required.
• Graduate of an accredited nursing school.
• 1–2 years of recent medical care experience; home health or hospice preferred.
• Current automobile insurance and valid Oregon driver’s license.
• CPR certification required.
• Strong communication and interpersonal skills; able to work independently and in a team.
• Compassion and dedication to patient care.

Responsibilities

• Perform initial comprehensive assessment to determine home health needs.
• Develop individualized care plans with goals and family involvement.
• Administer medications and treatments per physician orders.
• Update ongoing assessments and care plans as status changes.
• Coordinate care with case manager and healthcare team.
• Complete required OASIS assessments within specified timelines.
• Participate in on-call duties and after-hours visits as policy.
• Local travel by personal vehicle across Central Oregon.
