Job summary

Partners In Care (PIC) is a nonprofit provider delivering hospice, home health, and palliative care across Central Oregon, serving Deschutes, Crook, and Jefferson counties with a dedicated team of professionals. The Social Worker will offer psychosocial support within interdisciplinary teams and collaborate with families to align care with PIC’s mission.

The role requires a Master’s degree in social work, licensure or eligibility, and 1–3 years in end‑of‑life care, with travel across Central

Qualifications

• Master’s degree in social work (CSWE) or related field.
• Licensed or eligible for licensure (LMSW/LCSW/CSWA/LMFT/LPC/Psychologist).
• 1–3 years of end‑of‑life care experience preferred.

Responsibilities

• Perform comprehensive psychosocial assessments for home health, hospice and palliative care patients/families.
• Develop goals and interventions for the Interdisciplinary Plan of Care based on assessment findings.
• Provide professional social work services, including ongoing assessments, counseling, and referrals.
• Collaborate with the Interdisciplinary Care Team and other departments to ensure timely delivery of services.
• Work with Bereavement services to complete risk assessments and provide follow-up support.
• Travel throughout Central Oregon for fieldwork.
