Job summary

Partners In Care in Bend, Oregon seeks a Licensed Practical Nurse to join our home health and hospice team. You will provide direct nursing care under RN supervision, following care plans and ensuring patient comfort in their homes.

Responsibilities include med administration, patient/family education, and coordination with the care team. Travel across Central Oregon is required; valid Oregon driver’s license and reliable vehicle are necessary.

Qualifications

• Oregon LPN license in good standing.
• Minimum 1 year clinical experience; home health/hospice preferred.
• Must be 18+ years of age.
• Strong assessment, problem-solving, and nursing skills.
• Excellent communication, time management, and computer skills.
• Ability to pass background check, drug screen, and TB test.
• Valid CPR certification and Oregon driver’s license with reliable transportation.
• Able to work independently and with a team.
• Compassionate and patient-focused.

Responsibilities

• Provide direct patient care and follow the plan of care under RN supervision.
• Administer medications and treatments as prescribed.
• Educate patients and families about care needs.
• Maintain accurate EMR documentation per standards.
• Coordinate with the care team and attend care conferences.
• Visit patients at home and facilities as required.
• Adhere to policies and participate in ongoing education.
