Job summary

Partners In Care is seeking a Support Registered Nurse to plan, coordinate, and evaluate hospice services with potential cross-training in home health. The role emphasizes community-based nursing and collaboration with the patient’s family and interdisciplinary team.

The RN will conduct initial assessments, develop care plans, administer medications, and ensure timely documentation while supporting cost-effective, quality patient outcomes across Central Oregon.

Qualifications

• Must hold an unencumbered Oregon RN license.
• Background check, MVR, TB test and drug screen required.
• Graduate of an accredited nursing school.
• 1–2 years of home health or hospice experience preferred.
• Valid CPR certification required.
• Valid driver’s license and reliable transportation.
• Current automobile insurance is required.

Responsibilities

• Plan, coordinate, implement and evaluate hospice services; cross-train in home health.
• Develop individualized care plans with goals based on nursing diagnoses.
• Administer medications and treatments as prescribed by the physician.
• Complete and update ongoing assessments and care plans as patient status changes.
• Coordinate patient care with the case manager and healthcare team.
