Partners In Home Care (PIHC) is seeking a compassionate Registered Nurse to join our Hospice team serving Lake County and surrounding communities. This RN role focuses on delivering personalized hospice care at patients' homes, supporting families through end-of-life care, and collaborating with an interdisciplinary team.

The position offers a flexible schedule (full-time or part-time) with typical hours Monday–Friday, plus on-call rotations.

Qualifications

• Current and unrestricted RN license in Montana.
• ADN or BSN from an accredited program.
• Valid driver's license, auto insurance and reliable transportation; ability to travel throughout service area.
• Strong communication and organizational skills; ability to work independently within an interdisciplinary team.
• Certified Hospice and Palliative Nurse (CHPN) certification preferred.
• One year of experience in acute care, home health, hospice, palliative care, or related clinical setting preferred.

Responsabilités

• Provides comprehensive Hospice Care focusing on comfort, dignity, and quality of life for patients.
• Conducts thorough Patient Assessments to identify physical, emotional, and spiritual needs.
• Develops and implements individualized care plans addressing patient goals and preferences.
• Manages and alleviates symptoms through pain and symptom management strategies.
• Administers medications safely and per protocols.
• Offers bereavement support and practical guidance to families before and after a patient’s passing.
• Provides essential family support and education on patient care and the dying process.
• Collaborates with physicians, social workers, spiritual counselors, and other healthcare professionals as part of an Interdisciplinary Team.
• Educates patients and families on disease progression, treatment options, and available resources.
• Maintains accurate and timely patient records and documentation of assessments and outcomes.
