Job summary

Qualifications

• Graduate of National League of Nursing accredited school of nursing.
• Current licensure in State.
• Registered nurses shall have a minimum of (a) one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing from a program accredited by the National League of Nursing and a current RN license.
• Minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred.

Responsibilities

• Completes and maintains accurate clinical notes regarding patient condition and care.
• Communicates with physician regarding patient needs and changes in condition; obtains orders.
• Coordinates care with community health professionals to implement the care plan.
• Educates patient and family/caregiver on self-care, medications, and nutrition per physician instructions.
• Participates in revising the plan of care as patient needs change.
• Acts as Case Manager when assigned and coordinates patient care for caseload.
• Assists in providing continuity of care and a safe home environment.
• Supervises hospice aides and delegates duties as needed.
