Job summary

Three Oaks Hospice is seeking a registered nurse to provide skilled nursing care for patients in their homes, coordinating with the interdisciplinary team and families.

You will implement the care plan, educate patients and families about end-of-life care, perform on-call duties, maintain thorough documentation, and uphold ethical standards.

This role requires strong communication, compassion, and the ability to manage multiple priorities in a dynamic hospice environment.

Qualifications

• RN licensure in good standing with state requirements.
• Two years RN experience in clinical care; hospice/home health preferred.
• Current CPR certification.
• Proficient computer skills; Microsoft Office experience preferred.

Responsibilities

• Assess patient and family needs; coordinate care with the IDT.
• Implement the plan of care and revise as needed.
• Educate patient/family about disease, self-care, end-of-life care; train staff as needed.
• Initiate preventive and rehabilitative nursing procedures.
• Prepare progress notes showing goals achieved; document care.
• Coordinate services and prioritize needs with IDT; refer to other services as needed.
• Inform physician of changes in patient needs and outcomes.
• Determine scope and frequency of services based on acuity.
• Assess caregiver ability to meet needs; support as needed.
• Communicate using current organization processes and systems.
• Apply admission/re-certification criteria for hospice care.
• Perform on-call duties as assigned.
• Other duties as assigned.
