Three Oaks Hospice, Inc. seeks a compassionate Registered Nurse to deliver skilled nursing care to patients in their homes, coordinating with the interdisciplinary team and families to ensure comfort and quality end-of-life care.

In this on-site role, you will perform assessments, implement the plan of care, educate families, and document progress while meeting regulatory standards. CPR certification, a valid state license, and prior hospice or home health experience preferred.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ experience as an RN in a clinical care setting; hospice or home health preferred.
• Current CPR certification.
• Proficient computer skills; Microsoft Office experience preferred.
• Strong interpersonal and organizational skills; ethical practice.

Responsabilités

• Administer skilled nursing care for clients in their place of residence.
• Coordinate care with the interdisciplinary team, patient/family, and referring agency.
• Provide training and education to patient/family regarding disease process and end-of-life care.
• Initiate preventive and rehabilitative nursing procedures as needed.
• Prepare progress notes showing goal attainment and care outcomes.
• Coordinate services and prioritize needs with IDT members.
• Communicate changes in patient needs to physicians and team.
