Job summary

Elevation Hospice of Colorado seeks an experienced RN to provide skilled nursing care to patients in their homes in the Boulder area. You will coordinate with the interdisciplinary team, document care, and support patients and families through the end-of-life journey.

This full-time role requires an active RN license, CPR certification, and hospice experience, with strong communication and compassionate patient care. Join a dedicated team delivering best-in-class hospice services.

Qualifications

Graduate of an accredited Associate or bachelor’s degree in Nursing.
Current state license as an RN and current state driver’s license.
Two years’ experience as an RN in a clinical care setting; hospice/home health experience preferred.
Current CPR certification.
Proficient computer skills; Microsoft Office experience preferred.

Responsibilities

Administer skilled nursing care for clients in residence, coordinating care with the IDT.
Implement and revise the plan of care as needed; maintain progress notes.
Educate patients and families on disease processes, end-of-life care, and self-care techniques.
Initiate preventive and rehabilitative nursing procedures.
Coordinate services with patient/family and IDT; prioritize needs.
