Job summary

Elevation Hospice of Colorado is hiring a registered nurse to provide skilled nursing care in patients' homes. You will coordinate with the interdisciplinary team, assess needs, and support patients and families at end of life.

The role requires RN licensure, CPR certification, and 2 years of clinical RN experience, with hospice/home health preferred. On-site work with a Monday–Friday schedule and on-call responsibilities are included.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ experience as an RN in a clinical care setting; hospice/home health experience preferred.
• Current CPR certification.
• Proficient with computer use; Microsoft Office experience preferred.
• Strong interpersonal, communication, and organizational skills.

Responsibilities

• Perform initial and ongoing comprehensive assessments of patients.
• Implement and revise the plan of care (POC) as needed.
• Educate patients and families about disease, end-of-life care, and self-care techniques.
• Provide on-call nursing coverage as assigned.
• Coordinate care with the interdisciplinary team and referrals.
• Document progress notes demonstrating goal attainment.
• Assess caregiver capacity and patient needs at admission and throughout care.
• Communicate changes to the physician and team; adjust care as needed.
• Ensure appropriate levels of care and eligibility for hospice services.
