Job summary

Croí Health is seeking a Hospice Liaison to join our mission-driven team in Massachusetts. This full-time, 40-hour-per-week role focuses on guiding new referrals, coordinating with physicians, hospitals and payors, and ensuring safe and appropriate care for patients from referral to admission.

You will build strong relationships with referral sources, educate on services, and drive admissions growth. Compensation ranges from $75,000 to $90,000 annually, plus bonus potential based on credentials

Qualifications

• 5+ years of proven sales or business development experience.
• Bachelor’s degree preferred.
• Strong relationship-building, networking, and communication skills.
• Self-motivated, persistent, and comfortable with prospecting and face-to-face sales.

Responsibilities

• Develops, cultivates, and maintains relationships with physicians, hospitals, SNFs, rehabilitation centers, social workers, case managers, discharge planners, referral sources, patients, and families to generate new referral opportunities and drive admissions growth.
• Identifies, develops, and converts new referral sources; creates and executes outreach strategies to increase referrals, admissions, market share, and census.
• Serves as a key liaison between referral partners, patients, families, and Agency leadership to ensure a seamless referral and admission experience.
• Educates referral sources, patients, and families on services, capabilities, differentiators, and value proposition.
• Proactively conducts sales calls, facility visits, physician office visits, community outreach, and networking events to establish new relationships and strengthen partnerships.
• Develops and maintains a strategic referral network to maximize referral potential.
• Collaborates with Intake and clinical teams to assess referral opportunities and coordinate timely admissions.
• Maintains communication with referral sources throughout referral and admission process to ensure exceptional service.
