Job summary

Heartland Hospice is seeking a Hospice Care Consultant (HCC), a strategic sales representative who builds lasting relationships with referral partners in the healthcare community. You will educate, inform, and inspire confidence in hospice services to ensure timely referrals and the right patients are connected at the right time.

Qualifications

• Bachelor’s degree or equivalent experience required.
• Minimum 2 years in healthcare sales or referral marketing.
• Strong relationships with physicians, hospitals or SNFs.
• Excellent communication and time-management skills.
• Experience with hospice or home health is a plus.

Responsibilities

• Educate, inform, and inspire confidence in hospice services to ensure timely referrals.
• Identify new referral opportunities by calling on hospitals, home health agencies, SNFs, clinics, and physician offices.
• Engage in conversations with healthcare professionals about patient needs and hospice benefits.
• Plan and execute strategic marketing initiatives to drive referral growth.
• Represent the organization at community events and professional associations.
• Serve as a resource for patients and families, explaining hospice options.
