Job summary

ViaQuest Hospice in Muncie, IN seeks a Hospice Community Liaison to build and maintain strong referral partnerships, educate the community on hospice services, and ensure patient access to care. The role emphasizes strategic outreach to healthcare providers, hospitals, SNFs, and community groups, with a focus on ethical referrals and compassionate service.

You will meet referral goals, manage a CRM, and collaborate with clinical teams to ensure seamless patient transitions and high-quality care

Qualifications

• 2+ years of sales experience; healthcare experience preferred but not required
• Resilience and adaptability; ability to handle objections and rejection
• Growth mindset and willingness to learn and grow professionally
• Strong communication and relationship-building with healthcare professionals and families

Responsibilities

• Develop and maintain relationships with healthcare providers, hospitals, SNFs, assisted living facilities, and other referral sources
• Identify new referral opportunities and strategically grow the network
• Educate professionals, families, and community about hospice services and early referrals
• Manage CRM to document referrals, follow-ups, and activities
• Collaborate with clinical teams to ensure smooth transitions to hospice care
• Participate in patient and family follow-ups to ensure quality care
