Job summary

Transitions Hospice - Indiana is seeking a Care Coordinator to grow referrals across Fort Wayne and surrounding areas. In this role, you will be the face of our organization in your community, building trust with physicians, discharge planners, and healthcare providers to ensure patients receive compassionate hospice care.

You will manage the referral pipeline, educate accounts, and collaborate with the interdisciplinary team to resolve issues and support smooth patient transitions.

Qualifications

• Bachelor's degree and healthcare knowledge preferred.
• 3+ years of sales experience, ideally in healthcare.
• Proven ability to build customer relationships that drive referrals.

Responsibilities

• Generate referrals and admissions within your territory.
• Maintain strong relationships with physicians, nursing homes, hospitals, discharge planners, and case managers.
• Represent Transitions Hospice professionally across community interactions.
• Deliver educational in-services to accounts about hospice care.
• Implement marketing and promotional initiatives and manage territory growth.
• Document activity in SAM and the RHC Activity Sheet per company requirements.
• Collaborate with the Patient Care Manager to support seamless patient transitions.
• Participate in evening and weekend on-call admissions as required.
