Job summary

Heartland Hospice is seeking a Hospice Care Consultant to lead strategic outreach and build referral networks in the healthcare community. You will educate providers, identify opportunities, and advocate for patients needing hospice care, driving early referrals and high-quality end-of-life support.

The role emphasizes relationship-building, consultative marketing, and collaboration with clinical and operations teams to improve patient outcomes.

Qualifications

• Bachelor's degree in business, marketing, communications or equivalent experience.
• Minimum 2 years in healthcare sales, business development, or referral marketing.
• Strong existing relationships with physicians, hospitals, SNFs or home health professionals.
• Prior hospice/palliative care or home health experience is a plus.
• Excellent communication, presentation, and time-management skills.
• Proficiency in MS Office and CRM platforms.
• Sensitivity to the needs of terminally ill patients and families.
• Proven track record of meeting or exceeding admissions and census targets.

Responsibilities

• Educate, inform, and inspire confidence in our services so that referrals occur at the right time.
• Identify new referral opportunities by calling on hospitals, home health agencies, SNFs, and clinics.
• Engage healthcare professionals about patient needs and hospice benefits.
• Plan and execute strategic marketing initiatives to increase referral growth.
• Represent the organization at community events, associations, and in-services.
• Serve as a resource and advocate for patients and families regarding hospice options.
