Job summary

The Care Team Hospice Care is seeking a driven Community Care Liaison (CCL) to grow our presence in Indiana. This role focuses on building relationships with physicians, hospitals, and other referral sources to increase awareness, trust, and utilization of hospice services.

You will be the face of our hospice in the community, educating providers and families about the value of comfort-focused, patient-centered care.

Qualifications

• 1-2 years healthcare outside sales experience.
• 1-2 years Hospice or Home Health sales preferred.

Responsibilities

• Build and expand relationships with physicians, healthcare partners, and key decision-makers to drive referral volume and market growth.
• Serve as a dedicated advocate for hospice by educating the community and sharing the value of comfort-focused, patient-centered care.
• Achieve census and admission targets by positioning our hospice as a preferred provider in the community.
• Engage regularly with healthcare providers and referral sources to create strong, collaborative partnerships focused on patient well-being.
• Lead hospice discussions with patients and families, communicating services clearly with professionalism and empathy.
