Job summary

Gentiva in Georgia is seeking a Hospice Care Consultant to drive strategic referral growth and build lasting relationships with hospitals, home health agencies, SNFs, clinics, and physician offices.

You will educate partners about hospice options, identify opportunities, and advocate for patients and families to ensure timely referrals and quality end-of-life care.

Qualifications

• Bachelor's degree in business, marketing, communications or equivalent experience.
• Minimum 2 years in healthcare sales, business development, or referral marketing.
• Strong existing relationships with physicians, hospitals, skilled nursing facilities or home health professionals.
• Prior experience in hospice, palliative care, or home health is a plus.
• Excellent communication, presentation, and time-management skills.
• Proficiency in Microsoft Office Suite and CRM platforms.
• Sensitivity to the needs of terminally ill patients and their families.
• Proven track record of meeting or exceeding admissions and census targets.

Responsibilities

• Educate, inform, and inspire confidence in our services, so referrals are timely.
• Identify new referral opportunities by calling on hospitals, home health agencies, SNFs, clinics, and physician offices.
• Engage in meaningful conversations with healthcare professionals about patient needs and hospice options.
• Plan and execute strategic marketing initiatives to increase awareness and drive referral growth.
• Represent our organization at community events, associations, and in-services to promote services.
• Serve as a resource and advocate for patients and families, ensuring understanding of hospice options.
