Job summary

Gentiva is seeking a Hospice Care Consultant to lead referral growth by building relationships with hospitals, home health agencies, SNFs, and physician offices in the Georgia region. You will educate providers on hospice options and advocate for early referrals to improve patient quality of life.

The role focuses on strategic marketing, meeting admissions targets, and collaborating with clinical and operational teams to deliver compassionate care.

Qualifications

• Bachelor's degree or equivalent experience required.
• Minimum 2 years in healthcare sales, business development, or referral marketing.
• Strong existing relationships with physicians, hospitals, SNFs or home health professionals.
• Prior hospice, palliative care, or home health experience is a plus.
• Excellent communication, presentation and time-management skills.
• Proficiency in Microsoft Office Suite and CRM platforms.
• Sensitivity to needs of terminally ill patients and families.
• Proven track record of meeting or exceeding admissions and census targets.

Responsibilities

• Educate, inform and inspire confidence in our services to ensure right referrals at the right time.
• Identify new referral opportunities by calling on hospitals and various care facilities.
• Engage in meaningful conversations with healthcare professionals about patient needs.
• Plan and execute strategic marketing initiatives to drive referral growth.
• Represent Gentiva at community events and professional associations.
• Serve as a resource and advocate for patients and families regarding hospice options.
