Job summary

Gentiva is seeking a Hospice Care Consultant to lead referral growth and advocate for patients and families. You will develop and nurture relationships with hospitals, home health agencies, SNFs, and clinics, educating healthcare professionals about hospice options and benefits.

The role requires strong communication, strategic outreach, and the ability to collaborate with clinical and operational teams to ensure timely, appropriate referrals.

Qualifications

• Bachelor's degree in business, marketing, communications or equivalent experience.
• Minimum 2 years in healthcare sales, business development, or referral marketing
• Strong existing relationships with physicians, hospitals, skilled nursing facilities or home health professionals
• Prior experience in hospice, palliative care, or home health is a plus
• Excellent communication, presentation, and time-management skills
• Proficiency in Microsoft Office Suite and CRM platforms
• Sensitivity to the needs of terminally ill patients and their families
• Proven track record of meeting or exceeding admissions and census targets

Responsibilities

• Educate, inform, and inspire confidence in hospice services to ensure referrals at the right time.
• Identify new referral opportunities by calling on hospitals, home health agencies, SNFs, assisted living communities, clinics, and physician offices.
• Engage in meaningful conversations with healthcare professionals about patient needs and hospice benefits.
• Plan and execute strategic marketing initiatives to increase awareness and drive referral growth.
• Represent the organization at community events, professional associations, and educational in-services.
• Serve as a resource for patients and families, ensuring they understand hospice options.
