Job summary

Gentiva Hospice is seeking a Hospice Care Consultant to champion referrals and build lasting partnerships with hospitals, home health agencies, SNFs, and clinics. You will educate clinicians on our hospice services and coordinate with clinical and operations teams to optimize patient referrals.

Qualifications

• Bachelor's degree in business, marketing, communications or equivalent experience.
• Minimum 2 years in healthcare sales, business development, or referral marketing.
• Strong existing relationships with physicians, hospitals, SNFs, or home health professionals.
• Prior experience in hospice, palliative care, or home health is a plus.
• Excellent communication, presentation, and time-management skills.
• Proficiency in Microsoft Office Suite and CRM platforms.

Responsibilities

• Educate, inform, and inspire confidence in our services to drive referrals
• Identify new referral opportunities by calling on hospitals, SNFs, clinics, and offices
• Engage in meaningful conversations about patient needs and hospice support
• Plan and execute marketing initiatives to increase referral awareness
• Represent Gentiva at events and professional associations to promote services
• Serve as a resource for patients and families about hospice options
