Responsibilities

• Serve as the primary liaison between the hospice agency and local healthcare providers to guide patients to hospice services.
• Build and maintain relationships with hospitals, physicians, case managers, and referral partners.
• Generate new hospice referrals within assigned territories and existing accounts.
• Educate healthcare professionals about hospice services and patient eligibility.
• Coordinate with internal clinical teams to ensure seamless patient transitions into hospice care.
• Identify new market opportunities and strengthen community partnerships.
