Job summary

Agape Care Group is seeking a Hospice Account Executive to connect our hospice services with local providers and communities. You will serve as the primary liaison between our agency and hospitals, physicians, case managers and referral partners to ensure patients receive compassionate end-of-life care when it is needed most.

This field-based role focuses on building trusted relationships, generating referrals in assigned territories, educating providers about eligibility, coordinating care

Qualifications

• 2 years of sales experience in a clinical care setting; hospice preferred.
• Associate degree or health-related work experience required.
• Field-based position requiring reliable transportation and daily interaction with providers and patients.

Responsibilities

• Serve as the primary liaison between the hospice agency and local healthcare providers.
• Generate new hospice referrals within assigned territories and existing accounts.
• Educate healthcare professionals about hospice services and patient eligibility.
• Coordinate with internal clinical teams to ensure seamless patient transitions into hospice care.
• Identify new market opportunities and strengthen community partnerships.
• Maintain accurate referral documentation and communication with care teams.
