Job summary

Enhabit Home Health & Hospice is seeking a dedicated professional to manage relationships with referral sources and to educate partners about Enhabit services. You will help navigate post-acute care transitions, drive referral growth, and contribute to patient outcomes and satisfaction.

The role requires a Bachelor's or higher degree in healthcare-related fields, 2+ years in healthcare business development, and a valid driver license.

Qualifications

• Bachelor's degree or graduate of nursing, therapy or social work; equivalent work experience considered.
• Minimum two years in healthcare business development, community relations, marketing or sales.
• Knowledge of federal/state regulations for Medicare home health/hospice operations.
• Valid driver license and automobile liability insurance per law.
• Strong interpersonal, communication and presentation skills; clinical license if required.

Responsibilities

• Serve as primary relationship manager between Enhabit and referral sources (physicians, hospitals, clinics).
• Support referral growth by educating partners on Enhabit services and program requirements.
• Help navigate post-acute care transitions to improve patient outcomes and satisfaction.
• Promote Enhabit quality metrics and ensure strong referral source relationships.
