Job summary

Idaho Home Health and Hospice, part of the LHC Group within the Optum family, is seeking a Patient Care Manager - Hospice to support Executive leadership with safe, compliant patient care delivery. The role focuses on quality, safety, and regulatory adherence in home hospice settings.

Required RN licensure and driving ability, with CPR preferred. The position offers a comprehensive benefits package and opportunities for professional growth within a supportive care-focused organization.

Qualifications

• Current unrestricted RN licensure in state of practice.
• Current driver’s license and access to reliable transportation.
• CPR certification or ability to complete within 90 days of hire.
• 4+ years full-time RN experience.
• 1+ years clinical experience.
• Ability to work independently.
• Good communication, writing and organizational skills.

Responsibilities

• Ensure compliance with hospice policies and local/state/federal regulations.
• Direct and promote quality of care and patient safety for the team.
• Engage in thorough problem resolution and investigate complaints.
• Provide in-services to staff on regulatory issues and agency policy.
• Function effectively in any home setting regardless of client demographics.
