Job summary

UnitedHealth Group's Idaho Home Health and Hospice of Treasure Valley is seeking a Registered Nurse Case Manager to manage the plan of care for hospice patients in a compassionate, patient‑centered environment.

You will evaluate patients, coordinate care with the team, and ensure quality outcomes while navigating diverse patient needs and ensuring compliance with regulations. This role offers a strong salary range and comprehensive benefits.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• 1+ years of clinical experience.
• Current CPR Certification.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.

Responsibilities

• Perform initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Assess needs and re-assess every 15 days and update plan of care.
• Initiate and coordinate the plan of care and treatment strategies.
• Document problems, goals, interventions, and patient/family responses.
• Collaborate with patient/family, physician, and IDG members daily.
• Instruct and supervise patient/family in self-care when appropriate.
