Heart N Home Hospice, part of LHC Group and a member of the Optum family, is seeking a Registered Nurse Case Manager to oversee the Plan of Care for hospice patients at home. You will evaluate care needs, develop treatment strategies, and collaborate with physicians and the IDG to promote physical, psychosocial, and spiritual well-being.

Requirements include active RN licensure in the state, a valid driver's license and access to reliable transportation.

Qualifikationen

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license with valid vehicle insurance.
• Access to a dependable vehicle or public transportation.
• Preferred: 1+ years of clinical experience.
• Preferred: CPR certification or ability to complete within 90 days of hire.

Aufgaben

• Make the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
• Identify physical, psychosocial, and environmental needs and reassess at least every 15 days.
• Initiate and coordinate the plan of care.
• Document problems, goals, interventions, and patient/family response.
• Collaborate with patient/family, attending physician, and IDG to provide care daily.
• Instruct and supervise patient/family in self-care techniques when appropriate.
• Maintain accurate clinical notes on the patient's condition.
