Job summary

Optum is seeking an experienced Hospice Patient Care Manager - RN to supervise clinical services and coordinate care for home health patients within a defined area. You will lead an interdisciplinary team, oversee nursing visits, assessments, and the plan of care, ensuring compliance with physician orders and state and federal regulations.

The role requires an active RN license, reliable transportation, and strong communication and organizational skills.

Qualifications

• Current, unrestricted RN licensure in state of practice.
• Current driver’s license, vehicle insurance, and access to dependable transportation.

Responsibilities

• Directly/indirectly supervises home health aides and LPNs, provides instruction, and assigns tasks.
• Provides clinical services within the scope of nursing practice per physician plan of care.
• Completes comprehensive assessments (OASIS), medication reconciliation, and initial nursing evaluations.
• Receives referrals, assigns clinicians, evaluates orders, and plans start of care visits.
• Coordinates determination of home health benefits and insurance approvals.
• Ensures care is individualized, meets eligibility criteria, and aligns with physician orders.
• Reviews assessments and plans daily and consults clinicians with recommendations.
• Function with sensitivity to patients of all ages, races, and backgrounds.
