Job summary

Baptist Trinity Hospice, part of the LHC Group within the Optum family, seeks a Patient Care Manager to support executive leadership in delivering safe, efficient patient care. The role requires occasional travel to patients in the community and oversight of an interdisciplinary hospice team.

Primary responsibilities include regulatory compliance, quality and safety leadership, staff education, and effective problem resolution. RN licensure and driving need are required; CPR is preferred.

Qualifications

• Current unrestricted RN licensure in state of practice.
• Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
• CPR certification (preferred) or ability to obtain within 90 days of hire.
• 4+ years full-time RN experience.
• 1+ years of clinical experience.
• Ability to work independently.
• Good communication, writing, and organizational skills.

Responsibilities

• Ensure compliance with hospice policies and all regulatory requirements for the assigned team.
• Direct and promote quality of care and patient safety for staff and patients.
• Engage in problem resolution and complaint investigation.
• Provide staff in-services on regulatory issues, agency policy, and safety.
• Function effectively in diverse home care settings.
