Job summary

The Idaho Home Health and Hospice of Treasure Valley, part of the LHC Group within the Optum family, is hiring an RN Case Manager to deliver the Plan of Care for hospice patients. You will collaborate with the IDG and family to promote physical, psychosocial, emotional, and spiritual well-being.

The role requires an unrestricted RN license in Idaho, CPR certification, and at least 1 year of clinical experience.

Qualifications

• Current and unrestricted RN licensure in Idaho.
• Minimum of 1 year of clinical nursing experience.
• CPR Certification is required.
• Valid driver's license and reliable vehicle or access to transportation.
• Ability to work in diverse home environments with empathy and professionalism.

Responsibilities

• Initial nursing evaluation to determine hospice eligibility within 48 hours.
• Identify patient/family needs and re-assess at least every 15 days.
• Initiate and coordinate the plan of care with the care team.
• Document care plans, goals, and patient responses.
• Collaborate with patient/family, physician, and IDG members daily.
• Instruct patient/family in self-care techniques when appropriate.
