Idaho Home Health and Hospice, part of LHC Group within UnitedHealth Group, seeks a Patient Care Manager - Hospice to support the Executive Director with safe, compliant home-based patient care in a dynamic hospice setting.

You will lead clinical teams, drive quality and safety, handle investigations, and deliver staff training. Requires RN licensure in-state, CPR preferred, and 4+ years RN experience with reliable transportation.

Qualifications

• Current unrestricted RN licensure in state of practice.
• Current driver's license and access to reliable transportation.
• CPR certification or ability to obtain within 90 days of hire.

Responsabilités

• Responsible for compliance with hospice policies and regulatory requirements for the team.
• Promotes quality of care and safety of patients and staff.
• Engages in problem resolution and complaint investigation.
• Provides in-services to staff to address educational needs and regulatory issues.
• Function in any home setting regardless of client demographics.
