Job summary

Gentiva's Heartland Hospice is seeking a Patient Care Manager to lead clinical operations, oversee patient care, and coordinate visits in a home-based hospice setting.

The role requires an RN license, 3 years of direct patient care (2 in hospice/home health), and knowledge of Medicare/Medicaid, JCAHO/ACHC requirements. BSN preferred; comprehensive benefits support professional growth and compassionate care delivery.

Qualifications

• Current RN license in the state of employment.
• Minimum of 3 years direct patient care experience, including 2 years in a hospice or home-based care setting.
• Demonstrated knowledge of Medicare/Medicaid, JCAHO, ACHC requirements.

Responsibilities

• Oversee and direct patient care services to ensure high-quality hospice delivery.
• Manage visit scheduling and prioritization based on clinical judgment.
• Triage incoming referrals and medical concerns in a timely, compassionate manner.
• Lead Interdisciplinary Group (IDG) meetings and document minutes.
• Mentor and train associates to support development, performance, and compliance.
• Ensure care delivery meets regulatory standards and company policies.
