Job summary

Gentiva Hospice is seeking a dedicated Patient Care Manager to oversee clinical operations and ensure the delivery of top-quality hospice services. You will lead a diverse care team, coordinate care plans, and mentor staff while maintaining regulatory compliance.

This role reports to the Executive Director or Administrator and emphasizes compassionate leadership and performance improvement. The position requires RN licensure, a nursing degree, and at least 3 years of direct patient care,

Qualifications

• RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum of 3 years of direct patient care experience, including 2 years in hospice or home-based care.
• Demonstrated ability to apply hospice principles, industry standards, and regulatory compliance (Medicare, Medicaid, JCAHO, ACHC).
• Knowledge and compassion for terminally ill patients and their families.
• Ability to lead a diverse care team while adapting to emotional and clinical needs of patients.
• Strong interpersonal, organizational, and leadership skills.
• Competency in clinical documentation and technology systems.

Responsibilities

• Oversee and direct patient care services to ensure high-quality hospice care.
• Manage visit scheduling, including reassignment, missed, declined, or rescheduled visits based on clinical judgment.
• Triage incoming phone referrals and medical concerns promptly and compassionately.
• Lead or facilitate Interdisciplinary Group (IDG) meetings with patient updates and care plans.
• Mentor, train, and provide clinical support to associates for development and compliance.
• Ensure care delivery meets federal, state, and local regulatory standards and company policies.
• Support performance improvement initiatives and operational efficiencies.
