Job summary

Gentiva Hospice is seeking a Patient Care Manager to lead clinical operations and ensure compliant, high-quality hospice services. The role reports to the Executive Director or Administrator and focuses on coordinating care, mentoring staff, and maintaining regulatory standards.

The position requires an active RN license in the state of employment, with at least 3 years of direct patient care experience (2 in hospice/home care).

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum 3 years of direct patient care, incl. 2 years in hospice or home-based care.
• Knowledge of Medicare/Medicaid, JCAHO, ACHC regulatory standards.
• Ability to lead a diverse care team and adapt to emotional/clinical needs.

Responsibilities

• Oversee and direct patient care to ensure high-quality hospice services.
• Coordinate visit scheduling and clinical priorities.
• Triage referrals and medical concerns promptly and compassionately.
• Lead IDG meetings and document minutes.
• Mentor and support associates on performance and compliance.
• Ensure compliance with federal, state, and local regulations and policies.
• Support performance improvement and operational efficiencies at the location.
