Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum 3 years direct patient care, incl. 2 years hospice/home health.
• Knowledge of hospice principles, regulations, and compliance.

Responsibilities

• Oversee and direct patient care to ensure quality hospice delivery.
• Manage visit scheduling and clinical triage based on priority and judgment.
• Triage referrals and medical concerns promptly and compassionately.
• Lead IDG meetings: updates, care plans, minutes documentation.
• Mentor and support associates for development and compliance.
• Ensure care meets regulatory standards and company policies.
• Support performance improvement and operational efficiency at location.
